Little fixes to make the game more realistic.
- Clinch against the cage.
- Pushing off the cage.
- Southpaw
- Checking leg kicks
- Variations in punching/kicking... different animations as match goes longer, fighter gets tired?
- More "trademark" moves.
What else?

the patch made is a lot less realistic. people complained about flash KOs (the short elbow flash KOs were the only thing that needed to be taken out) and they decreased them dramatically. i've hit people with literally 10 flush head kicks in a round and not KO'd them. in reality it only takes one.

It is a game though, and it is pretty easy to land a head kick. If it only took one in the game, it would be horrible.
As for the TS suggestions, they had planned to make the cage a big part of the game, but it is easier said than done unfortunately. Southpaw was causing problems with the engine and they weren't able to correct them in time for the release and did not want to push the game back even further. They deemed it more important to get the game out than have a handful of fighters fighting southpaw. As for checking leg kicks, you can do it, there just isn't an animation for it.
Remember, this was their first attempt and no one else out there had really made a decent MMA game at this point, so it was one hell of a first installment in my opinion. I am just glad the gameplay is solid. Next year we will likely have a lot more animations, but as I said, this was their first attempt and they were not doing well financially.

When you are rocked standing you fighter should stumble around, not stand there in a perfect stance as if nothing happened. It would be a pretty sweet change I think.

Try doing player matches. More people are willing to stand and can't/don't just throw one punch and hold the right stick

They are not that easy to pull off if you are unpredictable on the ground. The problem is a lot of people just transition spam, or use the same patterns and make it easy to reverse.

also it's hard to do the reversals with the lag, you almost have to guess. also it's risky. so many times i've been on a bottom with a guy on top of me in side control and i would time a flick perfectly but not get the reversal and end up mounted.
