In Nevada (which is the strictest State to be sanctioned in), the minimum sight requirement is 20/200. So you could actually get by, but there's no reason not to have PRK surgery. I wouldn't recommend just the basic laser surgery because some Commissions are starting to deny Licensing (Nevada definitely does and I've heard one or two others as well) for surgeries that require an incision and the folding of a skin flap within the eye. Their concern is the flap could be easily damaged by impact.
Also, your retinal status is more of a concern than anything else, so get with a good retinal specialist. I was born with myopia, which stresses my retinas more than normal, and was initially denied a License on account of a small hole in my right retina. Had to have that corrected, and now I'm Licensed with the stipulation that my retinal specialist has to see me annually to make sure it's not detaching.
