I played football in highschool and rugby in college and the worst injury I've ever inflicted to someone was the year I played Soccer as a Senior in Highschool. I played goalie and I hesitate to say was a "dirty" player but lets say I was fairly physical. So earlier in the game this guy on the other team had broken away and when I dove on the ball had given me a fairly hard kick to the side. Well it so happened that later in the game he went up to put a head on a corner kick at the same time I jumped up to grab it. I got to the ball first and decided to give him a little payback in kind with an elbow on my way down. Him jumping up meeting my elbow with all my bodyweight coming down proved to be a bit more powerful than I assumed. Split his head open from his ear through his eyebrow to right above his nose. 43 stictches and a concussion. I didnt even get ejected for it the ref called it accidental contact.
