It looks like you rock forward onto your toes at the end of the lift. Are you remembering to squeeze your ass cheeks to bring your hips forward?

EDIT: Your head angle is also quite severe. When you are at the top of the lift you are looking at the ceiling. This is probably causing you to hyperextend your back.

Ya I noticed that too... I notice I was rounding my back a couple weeks ago and focused too hard on not rounding my back and not enough on some other key points. Thanks for the feedback

-get your ass back more...this will help activate your glutes and hamstrings more
-DO NOT rise with your hips before the bar leaves the ground. You must push your legs through the ground. This is often a quad weakness.
-try to get your shins more vertical at the start...this goes hand in hand with ass back at the start
-dont round shoulders on the way down unless that's how you start and finish(ie that's your style)...upper back rounding is ok but you need to be consistant
-dont change view point as you lower the weight..continue to gaze at about the same point throughout...whether you use Ripptoes suggestion of looking at the floor 6 or so feet infront of you or some point on the wall...
-remember to get deep breath prior to start of each rep and push out with your abs...

Merill mentioned another good point...remember to squeeze your glutes throughout...as hard as you can...esp at the start and finish of each rep...this will help transfer a lot of the load from lower back to the glutes...
