I have a brain and two eyes, so yes, I can tell them apart. For those who say they don't recognize what a good-looking guy is, how do you groom yourself? Obviously there is some sort of ideal look that you're going for to attract the opposite (?) sex, so you must have some idea of what attractive looks like.

Sure facial symmetry, strong features and an athletic build is easy to spot in comparison to a guy whose features are out of proportion, crooked, physically out of shape or whatever.
I just googled a few of those top 10 bachelor/sexiest guy lists though, and I wouldn't have been able to pick probably half of them.

I can tell ugly and model good looks on guys most of the time but everything in-between is really hard to tell. Even with ugly I get it wrong sometimes. I hear the wife and her sister say some star is good looking and all the time I thought that the guy was average at best. Now when it comes to hot I have no clue how women really judge this, after all women say Mick Jagger and Jack Nicholson are hot and I have no clue what they are looking at besides money and power. Women swear that it’s not their money that they are attracted to and we all know they wouldn’t lie about something like that.

I should add that I'm sometimes surprised by who girls find attractive. I remember being really surprised that many found Fred Durst to be good looking, beyond the whole "he's a lead singer (?!) thing".

I can tell if a guy is good looking and have no problem discussing such matters with my wife. The difference between myself and a female is that I look at a man's face and body, whereas a female looks at his shoes, watch, cell phone, bank statements, etc.
