First guess would be your shoes. Try checking the wear patterns on the soles of your present pair to see which parts of your feet you're using most, then get a pair that best accommodates your style (overpronator, underpronator, etc. - check the Asics website for more info).
As far as form problems go I don't have much experience, but I don't see why it couldn't be self-corrected. I would suggest running slower (ON GRASS!) and paying specific attention to your posture and gait. If you're running like a spaz... stop.
And of course a sensible weights routine is always advisable. Try doing some sit-ups, light deadlifts and rotational exercises in order to get fully warmed up pre-run. I often find that just stretching the lower back doesn't work well enough as a warm-up, you need to get the blood flowing in that area with a bit of resistance movement.
If none of this fixes the problem, go see a doc.
