A fall like that is caused by the worst way to hit the hurdle to bad. The Stress of being at the Olympics likely did it to her. It looks like she hit the top of the hurdle with ankle and she dragged her foot over the top of the hurdle causing her to fall. It looks like her lead leg got to the ground first and her momentum pulled the hurdle and her down to bad.
