For sparring and training it can hurt.. A lot.
But that’s just something that comes with it.

In a real fight (streetfight, etc) I have never taken a knee, although I have broken someone's ribs with knees to the ribs, and they were completely gassed after that. Getting hit in the face with a punch or a kick when you have adrenaline going through you won't hurt until after. When I'm sparring im relaxed and more technical, so things hurt a lot more.
