The 'standard' boxing hook has a tendency to do this when it's thrown at a bad angle. Most coaches will teach you to throw the low hook(to the body) with your fist vertical so you can get the most extension. Then sometimes the boxer hears that he's supposed to 'dig up' into the body, and doesn't change the way he throws the punch.
If you're throwing the hook to the body with your fist vertical, and and going anywhere above a straight horizontal line, you should ask your coach for the proper method.
Make sure they're aware of the problem in your hands, also.
