I was a network administrator at one fairly large company a few years ago. My boss bought this software that did remote access, screenshots of your PC every few minutes, tracked web & IM traffic and keystroke logging. He made me deploy this software to specific people in the company and would sit back and watch them all day. He eventually got one guy fired for watching porn for 8 hours a day, 5 days a week. Of course he sat in his office for most of that time watching him with his door closed!
Now I work as a engineer/consultant for a very large IT consulting company and I would say that 99% of companies know very little about what you are doing online, but the 1% that does, knows everything and documents everything you do online at work.
